Athletic & Activity Clearance Procedure

In order to participate in extracurricular sports and activities, students must have the required paperwork on file.

The 2019-20 Athletic/Activity Packet which includes all of the required participation forms and instructions, information about student accident insurance, and form auto-fill capability can be downloaded here:

 

Procedure For Getting "Cleared" (complete the steps in order!):

  1. Print out and complete all required forms (see details below). Be sure that parent/guardian and student sign each form as needed.
  2. For all sports (including Dance teams), take Physical form to the Health Office and get "Emergency Card / Hazing Form" signed off there
  3. Take all forms and your Participation Donation (see details below) to the Student Store
 
Note: The Emergency Card / Hazing Form must be turned in for each sport or activity. All other forms only need to be turned in once  per school year.
 

 

PART 1: Required Forms For All Sports and Activities:

  • The Emergency Card and Hazing Pledge form completed and signed by student and parent/guardian. This is page 18 of the Athletic/Activity Packet. A new form is required for every sport or activity the student participates in.
  • Athletic/Activity Report form listing insurance coverage and signed by parent and student. All students participating in extra-curricular activities must have insurance. This is page 20 of the Athletic/Activity Packet. A student only needs to turn in this form once per school year regardless of the number of sports and activities.
  • Athletic and Co-Curricular Code of Conduct form signed by parent and student. This is page 30 of the Athletic/Activity Packet. A student only needs to turn in this form once per school year regardless of the number of sports and activities.
  • F-603 Voluntary Sports / Non-sports Waiver form signed by parent. This is page 32 of the Athletic/Activity Packet. A student only needs to turn in this form once per school year regardless of the number of sports and activities.
  • F-605 Voluntary Activity form signed by parent and student. This is page 35 of the Athletic/Activity Packet. A student only needs to turn in this form once per school year regardless of the number of sports and activities.
  • **NEW REQUIRED FORM ADDED OCTOBER 2019** PVPUSD Athletic Handbook Acknowledgement Form signed by both parent and student. This is page 36 of the Athletic/Activity Packet. A student only needs to turn in this form once per school year regardless of the number of sports and activities.
 
 
PART 2: ***ADDITIONAL FORMS REQUIRED For All Sports and Dance Teams (but not for Activities):
  • You must have a Physical Examination Report on file in the Health Office. This form must be signed by a physician and must be an original (no copies). Physical Forms are valid for one calendar year. This is page 19 of the Athletic/Activity Packet.
  • Concussion Information Sheet signed by parent. This is page 26 of the Athletic/Activity Packet. A student only needs to turn in this form once per school year regardless of the number of sports and activities.
  • Sudden Cardiac Arrest Sheet signed by parent. This is page 29 of the Athletic/Activity Packet. A student only needs to turn in this form once per school year regardless of the number of sports and activities.
  • Opioid Facts Sign-off Sheet - signed by parent and student. This is a new requirement beginning January 2019. A student only needs to turn in this form once per school year regardless of the number of sports and activities. This is page 23.
  

PART 3: Other Forms

  • Parents who plan on providing transportation to activities for other students besides their own must complete the Volunteer Driver registration on line. Information and links for registering as a volunteer driver are provided on pages 37 through 39 of the Athletic/Activity Packet. 

 

PART 4: Participation Donation Information

 
The suggested Participation Donations for the 2018-19 school year is calculated based on the number of students participating in the activities, the events scheduled for the activities, and the cost of officiating, competition fees and transportation. Each activity will have a different suggested amount based on these variables. Participation donations do NOT  include the coaching or adviser stipends. While the suggested participation donation is not mandatory, it is needed in order to keep all activities and clubs viable.
 
As always, the student's involvement in any given activity will not be curtailed or increased based upon any donation. The donation, whether made or not made, will be completely confidential, known only by the Student Store, and will never be shared with the coaching staff or activities advisors. The coaches/advisors will only be made aware of the bottom line so that he/she can make prudent financial decisions regarding his/her program(s).
