HEALTH OFFICE PASS: All students must have a pass from their teacher before he/she will be admitted to the Health Office (emergency excepted). Students visiting the Health Office during class time must first check in with their next-period teacher and obtain a pass.
OFF-CAMPUS PASSES: Passes for doctor and dentist appointments are issued by the
Attendance Office. A dated note from parents must accompany requests for passes. Students going home due to illness must sign out through the Health Office.
ILLNESS: Students wishing to leave campus due to illness or injury must obtain permission from their parents (or a person listed on their emergency card) before they can be released. A phone call will be made from the Health Office to obtain permission.
INJURIES AND ACCIDENTS: All injuries that occur during school or any school-sponsored activity and which require medical attention must be reported to the school nurse within 24 hours. Obtain a note from the physician for a Physical Education Exemption. CASTS AND CRUTCHES: All students wishing to attend school in a cast or using crutches must first obtain a release from their physician. The school nurse will then provide them with a re-admission slip to class. A pass to leave class early will be issued as needed.
PHYSICAL EDUCATION EXCUSES: All students requiring a temporary exemption must submit a note from their parent or their physician. An exemption of five or more days will be granted only if a physician’s note is received.
MEDICATION: Students will not be permitted to have in their possession or take any medication (prescription or over-the-counter) at school unless a physician and the parent complete the necessary form. Inhalers may be carried only if the physician provides written certification that this is necessary. Please obtain the required form from the Health Office or the school website. Medication brought to school must be in a properly labeled pharmacy container and stored in the Health Office. Only a one-month supply may be kept at school. School personnel may not dispense any medication to students unless a completed medication form is on file.
