TO ORDER YOUR FINAL TRANSCRIPT 

COMPLETE THE

SENIOR YEAR END SURVEY

ON MAIA LEARNING

 

BEFORE YOU BEGIN THE SURVEY, MAKE SURE YOU RECORD ALL THE COLLEGES YOU APPLIED TO IN TO MAIA LEARNING BEFORE YOU BEGIN YOUR SENIOR SURVEY (directions below).

MRS. ARICO VISITED ALL ENGLISH CLASSES TO GO OVER THIS IN JANUARY.

 

Once the Senior Survey is complete you have officially ordered your transcript and don't need to do anything else.

 

DIRECTIONS TO COMPLETE SENIOR YEAR END SURVEY & ORDER FINAL TRANSCRIPT

1. Log on to MAIA Learning (directions are below)
2. From Home Page/Dashboard click on SURVEYS
3. Complete SENIOR YEAR END SURVEY (about 5 minutes) 
4. As part of the survey you will be asked for the outcome decision for each school you applied to. Make sure to choose the correct outcome and make sure the school you have decided to attend is noted as ACCEPTED-ATTEND
5. To be included in Senior Destinations published by Pen Newspaper - the Survey is DUE BY MAY 15
6. When you have noted which school is "Accepted-Attend" you have ordered your Final Transcript.  There will NOT be a final transcript order button or notification to you.
7. You must complete the survey by JUNE 6th in order for your final transcript to be sent on TIME by JULY 1.

FINAL TRANSCRIPTS WILL NOT BE SENT UNTIL LATE JUNE AFTER FINAL SEMESTER GRADES ARE COMPLETED BY TEACHERS.

 

WE WILL UPDATE THIS PAGE TO ALERT YOU TO THE STATUS OF FINAL TRANSCRIPTS.

DIRECTIONS TO RECORD THE COLLEGES YOU APPLIED TO:

1. Log on to MAIA Learning (directions are below)
2. From Home Page/Dashboard click on UNIVERSITIES in the left hand tool bar
3. Click on APPLYING tab along the top
4. Use the ADD UNIVERSITY button in top right corner and input the college you applied to 
5. For each school added complete each box along the bottom where the school is listed. These will be:
   1. TYPE (of application)
   2. TERM (Fall 2026/Fall)
   3. ROUND (leave this as 1)
   4. PLAN (Early Decision, Early Decision 1, Early Decision 2, Early Admission, Restricted Early Admission, Regular Admission, Rolling Admission) this is based specifically on what the school offers
   5. DEADLINE (based on the colleges deadline for the plan you utilized)
   6. You can use the + button to add the program you applied to
   7. You MUST click on the MARK AS APPLIED button on the far right, click on YES I HAVE APPLIED in pop up. Then choose the date you applied. Choose ANY date before the deadline noted.
   8. Button in far right bottom corner will now be green APPLIED

You must complete this process for all the schools you applied to.

IF YOU WANT TO CHECK UP TO SEE IF YOU COMPLETED THE SENIOR YEAR END SURVEY -  FOLLOW THESE STEPS

1. Log on to MAIA Learning (directions are below)
2. From Home Page/Dashboard click on SURVEYS and the Senior Year End Survey should be noted as completed 
3. Click on UNIVERSITIES from the left hand menu, then choose DECISIONS from the menu across the top - Each school you applied to should note the final outcome and ONE should be noted as ACCEPTED-ATTEND.

TO CHECK WHICH SCHOOL YOUR FINAL TRANSCRIPT WILL BE SENT

1. Log on to MAIA Learning (directions are below)
2. From Home Page/Dashboard click on UNIVERSITIES in the left hand tool bar
3. Choose DECISION from the top menu. Each school you applied to should have the final outcome and the school you are attending in the Fall should be noted as ACCEPTED-ATTEND. This is the school where your transcript will be sent.

IF YOU NEED TO CHANGE YOUR FINAL SELECTION

1. Log on to MAIA Learning (directions are below)
2. From Home Page/Dashboard hover on UNIVERSITIES in the left hand tool bar
3. Choose  DECISIONS from the top menu. School noted  is where your transcript will be sent
4. Click ACTIONS and follow the prompts to add the correct final decision from your College Applications List
5. Send an email to aricoj@pvpusd noting the change and include documentation of the withdrawal from the original college/university. Please put the following in the subject line:  CLASS OF 2026 FINAL TRANSCRIPT DESTINATION CHANGE

 

Transcripts ordered AFTER JUNE 6th cannot be guaranteed to arrive by July 1.
